NEW YORK CITY VITAL RECORDS
The New York City Department of Health and Mental Hygiene, Office of Vital Records (New York City Vital Records) issues certified copies of New York City birth certificates, New York City death certificates and New York City marriage records, for events that occurred in the five boroughs of New York City (Manhattan, Brooklyn, Queens, the Bronx and Staten Island).

New York City death certificates may only be obtained in person or by mail. For address and pricing information, please call the New York City Department of Health and Mental Hygiene automated information line at 212.788.4520.

New York City marriage records may only be obtained in person or by mail. For address and pricing information, please contact the NYC Clerk at 212.669.2400.

New York City divorce records must be ordered through New York State Vital Records.

You may order copies of New York City birth certificates through VitalChek on an expedited basis.

Who Can Order?
You may order copies of New York City birth certificates for yourself or for a child, as long as your name appears on certificate*. Your name must also appear on the credit or debit card that is being used for payment.

Where Can The Document Be Shipped?
New York City Vital Records will only ship copies of New York City birth certificates to the entitled applicant's verified credit card billing address. Please note that express couriers will not ship to a P.O. Box.
