OREGON VITAL RECORDS
The Oregon Division of Vital Records (Oregon Vital Records) issues certified copies of Oregon birth certificates, Oregon death certificates, Oregon marriage records and Oregon divorce records for events that occurred in the State of Oregon. You may order copies of Oregon vital records through VitalChek on an expedited basis.

Identification Requirements
Oregon Vital Records requires all applicants to submit a copy of their government issued photo identification. Appropriate forms will be provided at the end of the order process.

OREGON BIRTH CERTIFICATES
Oregon Vital Records issues certified copies of Oregon birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes.

Who Can Order?
Access to copies of Oregon birth certificates (for events that occurred within the last 100 years) is restricted to the persons named on the document (certificate holder or parents), immediate family members and legal representatives (includes legal guardians or persons with power of attorney).

If you are a legal guardian or have power of attorney, you must enclose a copy of the legal document. If you are not eligible to order the record, please include a permission note with the notarized signature of an eligible person. Government agencies may order if the certificate is needed in the course of their work or to prevent fraud. When ordering a birth certificate, you must state your relationship to the person on the record; if you are an attorney, you must state whom you represent. If you do not legally represent the registrant, include how your client is related to the person on the record. Entitlement documentation will need to be faxed directly to Oregon Vital Records upon completion of your online order.


OREGON DEATH CERTIFICATES
Oregon Vital Records issues certified copies of Oregon death certificates that can typically be used to obtain death benefits, to claim insurance proceeds and for other legal purposes.

Access to copies of Oregon death certificates (for events that occurred within the last 50 years) is restricted to family members and their legal representatives, persons or organizations with a personal or property right and government agencies. When ordering a death, marriage or divorce record, you must state your relationship to the decedent, or your reason for needing the record. If you are a legal representative of a family member, include whom you represent and how they are related to the person named on the record. If you have a personal or property right, explain why you need the record. If you are not eligible to order the record, please include a permission note with the notarized signature of an eligible person. DEATH 10YR EXT
This certificate can be ordered by customers who are unsure of the exact date of event. You will need to enter 01 as the month, 01 as the day and the approximate year of event. If a record is found within the ten years searched, a certificate will be issued. If not, the application will receive a 'No record letter'. The search fee is non-refundable.

DIVORCE
A certified copy of the Certificate of Divorce. This is not a copy of the divorce decree. Divorce records are available for events that occurred within the State of Oregon from 1925 to present.

CERTIFICATE OF DOMESTIC PARTNERSHIP
The Certificate of Domestic Partnership is a commemorative certificate, suitable for display, which features an attractive design with calligraphy-like font, high-quality paper, and a State of Oregon seal. The Certificate of Domestic Partnership is not a legal document.

MARRIAGE
