UTAH VITAL RECORDS
The Utah Department of Health, Office of Vital Records and Statistics (Utah Vital Records) issues certified copies of Utah birth certificates, Utah death certificates, Utah marriage records and Utah divorce records, for events which occurred within the State of Utah. UTAH BIRTH CERTIFICATES
The State of Utah issues only certified copies of Utah birth certificates, which are typically acceptable for passport, social security, employment and personal identification purposes.
Who Can Order?
To obtain copies of Utah birth certificates, you must be the person on the record or an immediate family member. Immediate family is defined as a spouse, child, parent, sibling, grandparent or grandchild.
If you are the registrant's legal guardian, you must provide certified/stamped court approved documents. Documentation can be faxed to 866.760.1898 upon completion of your online order.
If you are a legal representative, (attorney, physician, funeral service director, genealogist, or other agent of the subject or subject's immediate family who has court documents) you must provide a letter, on business letterhead, stating why you are requesting the document. You will also need to submit backup documentation (court documents). Documentation can be faxed to 866.760.1898 upon completion of your online order.
