Birth Certificates

THE KENTUCKY OFFICE OF VITAL STATISTICS
The Kentucky Office of Vital Statistics issues certified copies of birth, stillbirth, death, marriage and divorce certificates for events that occurred within the Commonwealth of Kentucky.

Certificate Fees:
The Kentucky Office of Vital Statistics cannot by law, provide free information from records, perform a genealogical search or verify whether a certificate is on file without a completed application form and the required fee attached. A fee is required for certified copies or records; or for a search of the files or records when no copy is available. The Kentucky Vital Statistics office will, upon receiving the application and fee, perform a search of the record. If a certificate is on file, it will be mailed to you. The required fee covers the cost of the record search and no refunds are issued if a certificate is not found. The office does not issue non-certified copies or photocopies of certificates.

Who Can Order?

Certificate Availability:

Kentucky's Vital Statistics Law, enacted by the General Assembly in 1910, provides for and legalizes the registration of births. The Office of Vital Statistics does not have records of births prior to 1911, except delayed records of births for those born before 1911 which have been established by affidavits and documentary evidence.

The Kentucky Department for Libraries and Archives, P.O. Box 537, 300 Coffee Tree Road, Frankfort, KY 40602, (502) 564-8300, has birth records for the cities of Louisville, Lexington, Covington and Newport, which enacted records collection ordinances before 1911. KDLA also has birth records (statewide coverage) prior to 1911.
