The Arizona Office of Vital Records (Arizona Vital Records) issues copies of Arizona vital records including certified copies of Arizona birth certificates and Arizona death certificates for events that occurred within the State of Arizona. Arizona marriage records and Arizona divorce records are maintained in the county where the marriage or divorce occurred. Identification Requirements:

You will be presented with an Authorization Form at the end of your order that includes specific instructions. When faxing your identification, please enlarge and lighten to ensure your fax is legible. Your order will not be processed until a legible form of identification is received. Below is a list of acceptable forms of identification:

  • A state issued Driver's License that includes a photograph
  • A state issued picture identification
  • Valid Passport (full document must be on one page to ensure document’s validity)
  • Federal ID
