INDIANA VITAL RECORDS
The Indiana Division of Vital Records (Indiana Vital Records) issues certified copies of Indiana birth certificates and Indiana death certificates for events that occurred within the State of Indiana. You may order copies of Indiana vital records through VitalChek on an expedited basis.

Who Can Order?
Access to Indiana birth certificates is restricted to the certificate holder or an immediate family member - mother, father (if named on the record), spouse, adult child, adult grandchild, adult sibling, maternal grandparent, paternal grandparent (if father is named on the record),, maternal aunt or uncle, paternal aunt or uncle (if father is named on the record).
Important - Anyone not listed, including ex-spouse, legal guardian or legal representative, must submit proof of entitlement (i.e. guardianship papers, power of attorney, etc.). Entitlement documentation will need to be faxed to 866.559.9631 upon completion of your online order.
Where Can The Document Be Shipped?
Indiana Vital Records will ship copies of Indiana birth certificates to the entitled applicant's verified address.
