FLORIDA VITAL RECORDS
The Florida Department of Health, Office of Vital Statistics (Florida Vital Records) issues certified copies of Florida birth certificates, Florida death certificates, Florida marriage records and Florida divorce records for events which occurred in the State of Florida. You may order copies of Florida vital records through VitalChek on an expedited basis.

Florida Vital Records issues copies of certified Florida birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes.

Who Can Order?
You may order copies of Florida birth certificates for yourself or for a child, as long as your name appears on the birth certificate. If your name does not appear on the Florida birth certificate being requested, you must submit documentation showing a direct and tangible interest. If the certificate holder named on the Florida birth certificate is deceased, you must also submit a copy of the death certificate. Please fax this documentaion to 877.550.7428.

BIRTH - COMPUTER COPY
A certified computer generated copy of the original record with the embossed Seal of Florida. This certificate is accepted by all state and federal agencies as legal proof of birth and citizenship. It can typically be used for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes. The computer copy is available for events that occurred in the State of Florida from 1917 to present.

The book copy is available for events that occurred in the State of Florida from 1865 to present.
