ILLINOIS VITAL RECORDS
The Illinois Department of Public Health, Vital Records (Illinois Vital Records) issues certified copies of Illinois birth certificates and Illinois death certificates for events which occurred within the State of Illinois. You may order copies of Illinois vital records through VitalChek on an expedited basis.

ILLINOIS BIRTH CERTIFICATES
Illinois Vital Records issues certified copies of Illinois birth certificates, which are typically acceptable for passport, social security, employment, school registration, personal identification purposes and other legal purposes.

Who Can Order?
You may order copes of Illinois birth certificates for yourself, your spouse and your child. If you are the certificate holder's legal guardian or legal representative, you must submit written evidence of guardianship or legal representation.

BIRTH LONG
A certified photocopy of the original record with the embossed Seal of Illinois. This certificate is accepted by all state and federal agencies as legal proof of birth and citizenship. It can typically be used for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes. Birth Certificates are available for events that occurred in the State of Illinois from 1916 to present.

First Copy: 15.00   Additional Copies: 2.00


BIRTH SHORT
A certified abstract showing child's name, birth date, sex, county of birth, certificate number and file date. Depending on the year of birth, it may also list the parents' names. It can typically be used as proof of identity, but cannot be used to obtain a passport if the parents' names are not listed. The Birth Certificate - Short is available for events that occurred within the State of Illinois from 1916 to present.
