NEW YORK VITAL RECORDS
The New York State Division of Vital Records (New York State Vital Records) issues certified copies of New York birth certificates, New York death certificates, New York marriage records for events which occurred in the State of New York (except the five (5) boroughs of New York City) and New Hyde Park. New York State Vital Records issues copies of New York divorce certificates for events that occurred anywhere in the State of New York, including New York City. You may order copies of New York Vital Records through VitalChek on an expedited basis.

NEW YORK BIRTH CERTIFICATES
New York State Vital Records does not file and cannot issue copies of New York City birth certificates. For births that occurred in one of the five (5) boroughs of New York City (Manhattan, Kings (Brooklyn), Queens, The Bronx and Staten Island), you will need to place your order with the New York City Department of Health.
Who Can Order?
You may order copies of New York birth certificates for yourself or for a child, as long as your name appears on the certificate*. Your name must also appear on the credit or debit card that is being used for payment.
*If your name does not appear on the New York birth certificates being ordered, upon completion of your order, please fax a copy of your government issued photo I.D. and proof of entitlement to (877) 854-4607. See below for acceptable forms of documentation:
- Power of Attorney (less than 1 year old)
- Custody Papers (less than 1 year old)
- Guardianship Papers (less than 1 year old)
- Official letter from attorney, on letterhead, providing an explanation as to why the document is pertinent to their needs, along with a copy of the attorney's bar card. You will also need to include a notarized statement of release from the client (certificate holder).
- Government offices (Social Security Admin., Dept. of Human Services, etc.) must submit request in writing directly to New York State Vital Records.
Where Can The Document Be Shipped?
New York State Vital Records will only ship copies of New York birth certificates to the entitled applicant's verified credit card billing address.
P.O. Boxes: Please note that New York Vital Records will not ship copies of New York birth certificates to a P.O. Box. If you do not have a physical address, you will need to submit a written request directly to New York State Vital Records, along with a check or money order.
International Shipments: If you are requesting to have your order shipped to an international address outside the U.S., Canada, Mexico or American Territory, you will need to submit the following documentation: 1) a copy of your valid passport and 2) a copy of your valid government issued photo ID or two (2) utility bills in your name. The utility bills cannot be more than six (6) months old. Please fax the necessary documentation to (877)854-4607.

BIRTH A certified birth certificate that can typically be used for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes. Birth Certificates are available for events that occurred within the State of New York
(except New York City), from 1907 to June 4, 2008.
Important Note- New York State Vital Records will not ship a birth certificate to a P.O. Box.
