Who Can Order?
Amherst birth certificates are provided for persons over 18 years of age. A parent or guardian must request a copy of a minor's birth certificate. Proof of identification is required. The most commonly accepted form is a valid drivers license.

Copies of Amherst birth certificates for genealogical purposes are available if the birth occurred 75 years ago, unless the applicant is a lineal descendent or has been designated to act on behalf of a lineal descendant. A lineal descendant is a son, daughter, grandson or granddaughter.
