The Duval County Division of Vital Records (Duval County Vital Records) issues certified copies of Duval County birth certificates and Duval County death certificates, for events which occurred within Duval County. DUVAL COUNTY DEATH CERTIFICATES
Duval County Vital Records issues certified copies of Duval County death certificates of individuals who died within Duval County.   • To obtain a copy of a death certificate with the cause of death showing on the certificate, you have to be an immediate family member. This includes a spouse, brother or sister, son or daughter, grandparent or grandson or granddaughter;
  • Beneficiary of a deceased person’s life insurance policy (must provide documentation);
  • Estate administrator (must provide documentation).
    Without the cause of death listed:
  • Anyone may request a copy of a death certificate without the cause of death showing on the certificate.

A certified photocopy of the original death record, which includes cause of death information. This certificate is accepted by all state and federal agencies. DEATH WITHOUT CAUSE
A certified photocopy of the original death record with the embossed Seal of Florida. This death certificate is accepted by all state and federal agencies. It can typically be used to obtain death benefits, claim insurance proceeds, notify social security and other legal purposes. Death Certificates are available for events that occurred in Duval County from 1893 to present.
