Who Can Order?
Access to Indiana death certificates is restricted to the decedent's immediate family - mother, father, spouse, adult child, adult sibling, grandparent, aunt, uncle, adult niece, adult nephew or a direct descendent.
Important - Anyone not listed, including ex-spouse, legal guardian or legal representative, must submit proof of entitlement (i.e. guardianship papers, power of attorney, etc.). Entitlement documentation will need to be faxed to 866.559.9631 upon completion of your online order.
