PENNSYLVANIA VITAL RECORDS
The Pennsylvania Department of Health, Division of Vital Records maintains records of births and deaths that occurred in Pennsylvania from 1906 to the present. The Division of Vital Records issues only certified copies of birth and death records.

The Pennsylvania Division of Vital Records office does not issue copies of Pennsylvania marriage and divorce records. Copies of these vital records may be obtained by contacting the courthouse in the county where the marriage license was issued or divorce decree was granted.

PENNSYLVANIA DEATH CERTIFICATES
Pennsylvania Division of Vital Records issues certified copies of Pennsylvania death certificates with the raised seal that are typically acceptable for obtaining death benefits, Social Security benefits, estate settlement, insurance claims and for other legal purposes.

Who Can Order?
To order certified copies of Pennsylvania death certificates, the eligible applicant must be the legal representative of the decedent's estate, a member of the decedent's immediate family (spouse, parent, sibling, child, grandparent or grandchild), or an extended family member who can establish a direct relationship to the decedent.

Where Can The Document Be Shipped?
Pennsylvania Vital Records will only ship certified copies of Pennsylvania death certificates to the eligible applicant's verified credit card billing address.

NOTICE: The credit card used to place the order must be registered in the name of the eligible requestor and the certificate will only be mailed to the name and address as reflected on the credit card billing address.

The death record will be returned by either regular mail, or for an additional fee of $17.00, you my select the option of UPS Air delivery. A signature is required for UPS Air delivery. Please note that UPS Air will not ship to a P.O. Box.
