Who Can Order?
To order copies of Cook County death certificates, you must be a member of the decedent's immediate family. If you are not a member of the decedent's immediate family, you must submit documentation proving a financial interest in the individual's death.
Where Can The Document Be Shipped?
Cook County Clerk's Office will only ship copies of Cook County death certificates to the entitled applicant's home or work address. Please note that express couriers will not ship to a P.O. Box.
