MIDDLETOWN VITAL RECORDS
The City of Middletown (Middletown Vital Records) issues certified copies of Middletown birth certificates, Middletown death certificates, and Middletown marriage records for events which occurred within the City of Middletown. You may order copies of Middletown vital records through VitalChek on an expedited basis.

IDENTIFICATION REQUIREMENTS
All applicants are required to submit a photocopy of their identification. Identification should be faxed to (860) 344-3588 upon completion of your online order. Acceptable forms of identification must include two of the following, one of which must be a photo ID:

  • Social Security Card
  • Employment ID
  • State or CIty ID
  • If you do not have an acceptable form of identification (listed above), please submit a photocopy of at least two recent bills. Your name and current address must be visible on the photocopies.
