FLORIDA VITAL RECORDS

The Florida Department of Health, Office of Vital Statistics (Florida Vital Records) issues certified copies of Florida birth certificates, Florida death certificates, Florida marriage records and Florida divorce records for events which occurred in the State of Florida. You may order copies of Florida vital records through VitalChek on an expedited basis.

FLORIDA DEATH CERTIFICATES
Florida Vital Records issues copies of certified Florida death certificates, that can typically be used to obtain death benefits, to claim insurance proceeds, and for other legal purposes.
Who Can Order?
Anyone may order copies of Florida death certificates; however, Florida death certificates containing the cause of death information are only issued to the decedent's immediate family (parent, spouse, adult child, adult grandchild, adult sibling). If you are not a member of the decedent's immediate family, you must submit proof of entitlement demonstrating tangible interest in the estate of the decedent. Please fax this documentation to 877.550.7428. (Note: Cause of death becomes public information 50 years following the death. Prior to that time, cause of death information is considered confidential.)
