    Have the following on hand before you begin your order:

    • The state or province (and in most areas of the US, the city and/or county) in which the birth took place.
    • The certificate holder's name as it appears on the birth certificate. If adopted, please enter your adoptive information.
    • The month, day and year of the birth (and in some cases, the name of the hospital).
    • The legal name of the parents identified on the birth certificate (including the mother's maiden name before marriage).
    • Your billing and shipping addresses (street address only - courier deliveries will not be shipped to PO boxes, APO or FPO military addresses). Note that some agencies will only ship to the verified credit card billing address.
    • Your method of payment. We recommend having your valid credit card with number and expiration date in hand while you order.

    Note: In most instances, you must be a member of the birth certificate holder's immediate family to receive the record.

  2. Have the following on hand before you begin your order:

    • The state or province (and in most areas of the US, the city and/or county) in which the event occurred. In some cases, the funeral home name is also required.
    • The deceased holder's name as it appears on the death certificate. Some agencies may also require the name of the spouse or parents of the deceased.
    • The month, day and year of the event. In some cases, the social security number of the deceased is also required.
    • Your billing and shipping addresses (street address only - courier deliveries will not be shipped to PO boxes, APO or FPO military addresses). Note that some agencies will only ship to the verified credit card billing address.

    Have the following on hand before you begin your order:

    • The names of the bride and groom as they appear on the marriage license.
    • The month, day and year of the event.

    • Your method of payment - We recommend having your valid credit card with number and expiration date in hand while you order.

  4. Order A Divorce Certificate

    Have the following on hand before you begin your order:

    • The state or province (and in most areas of the US, the city and/or county) where the divorce was granted.
    • The names of the parties involved as they appear on the divorce decree.
    • The month, day and year of the event.
    • Your billing and shipping addresses (street address only - courier deliveries will not be shipped to PO boxes, APO or FPO military addresses). Note that some agencies will only ship to the verified credit card billing address.
    • Your method of payment. We recommend having your valid credit card with number and expiration date in hand while you order.

    Note: This is a divorce certificate, not a divorce decree. To obtain a copy of the divorce decree, please contact the County Clerk's Office in the county where the divorce was granted.
