MONTANA VITAL RECORDS
The Montana Division of Vital Records (Montana Vital Records) issues certified copies of Montana birth certificates and Montana death certificates for events which occurred within the State of Montana. You may order copies of Montana vital records through VitalChek on an expedited basis.

MONTANA BIRTH CERTIFICATES
Montana Vital Records issues only certified copies of Montana birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes.
Who Can Order?
An applicant may only order copies of his or her own Montana birth certificate or one for an immediate family member (parent, spouse, child (must provide proof of relationship/legal need), legal guardian (must provide proof of relationship) or an authorized representative (must provide proof).
Birth Search
Anyone can request a Birth Search, as long as the event occurred 30 years prior to the date of the application. If the exact date is unknown, the fee is $10.00 for the first five years searched and $1.00 per year over the first five years searched (per individual named). A Genealogy\Informational copy of the record will be issued if found or a "No Find" letter will be sent. Birth Search requests must be made by phone or fax.
