NEW YORK VITAL RECORDS
The New York State Division of Vital Records (New York State Vital Records) issues certified copies of New York birth certificates, New York death certificates, New York marriage records for events which occurred in the State of New York (except the five (5) boroughs of New York City) and New Hyde Park. New York State Vital Records issues copies of New York divorce certificates for events that occurred anywhere in the State of New York, including New York City. You may order copies of New York Vital Records through VitalChek on an expedited basis.

NEW YORK DEATH CERTIFICATES
New York State Vital Records does not file and cannot issue copies of New York City death certificates. For events that occurred in one of the five (5) boroughs of New York City (Manhattan, Kings (Brooklyn), Queens, The Bronx and Staten Island), you will need to place your order with the New York City Department of Health.
Who Can Order?
New York death certificates are only issued to the spouse, parent or adult child of the decedent*. The applicant's name must appear on the credit or debit card that is being used for payment.
*If you are not the spouse, parent or adult child of the decedent, you must submit documentation proving lawful right. (Government agencies must submit vital record requests in writing directly to New York Vital Records.)
Acceptable documentation is as follows:
- Letter of Testamentary - issued by a probate court or an office of the court informing an executor of a will of his/her appointment and empowering the executor to discharge the appointed responsibilities.
- Court Order - must verify the requested death record is required from the applicant in order to process a claim.
- Official Letter (from a bank, insurance company, etc.) - must verify the requested death record is required from the applicant in order to process a claim.
Where Can The Document Be Shipped?
New York State Vital Records will only ship copies of New York death certificates to the entitled applicant's verified credit card billing address.
International Shipments: If you are requesting to have your order shipped to an international address outside the U.S., Canada, Mexico or American Territory, you will need to submit the following documentation: 1) a copy of your valid passport and 2) a copy of your valid government issued photo ID or two (2) utility bills in your name.
