Who Can Order?
Oklahoma birth certificates are not open for public inspection. Access is restricted to the person named on the record, the immediate next of kin, or an authorized agent working in the best interest of the registrant. All applicants must submit a copy of their government issued photo identification. If you are requesting a copy of your spouse's certificate, you will also need to submit a copy of their photo ID, along with an authorization letter stating that you have permission to order and receive a copy of their birth certificate.

Birth Short Form Certificate
A certified birth certificate that can typically be used for personal identification and school registration. This certificate cannot be used for social security, travel, passport, legal purposes, etc. Birth Certificate short forms are available for events that occurred within the State of Oklahoma from October 1, 1908 to present.
