Identification Requirements

OKLAHOMA BIRTH CERTIFICATES
Oklahoma Vital Records issues only certified copies of birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes.
Who Can Order?
Oklahoma birth certificates are not open for public inspection. Access is restricted to the person named on the record, the immediate next of kin, or an authorized agent working in the best interest of the registrant. All applicants must submit a copy of their government issued photo identification. If you are requesting a copy of your spouse's certificate, you will also need to submit a copy of their photo ID, along with an authorization letter stating that you have permission to order and receive a copy of their birth certificate.
