TENNESSEE VITAL RECORDS
The Tennessee Division of Vital Records (Tennessee Vital Records) issues certified copies of Tennessee birth certificates, Tennessee death certificates, Tennessee marriage records and Tennessee divorce records for events that occurred within the State of Tennessee. TENNESSEE BIRTH CERTIFICATES
Tennessee Vital Records issues only certified copies of Tennessee birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes. The Birth Short Certificate cannot be used to obtain a passport.

Who Can Order?
An applicant may only order copies his or her own Tennessee birth certificate, or one for certain family members. A legal representative who represents the registrant, or a qualified member of the registrant's family, may also order copies of Tennessee birth certificates.

Where Can The Document Be Shipped?
Tennessee Vital Records will only ship copies of Tennessee birth certificates to the entitled applicant's address.

Birth Long
The Birth Long copy is an exact certified duplicate of the original record that can typically be used for travel, passport, proof of citizenship, social security, driver's license, school registration, personal identification and other legal purposes. Birth Certificate - Long forms are available for events that occurred in the State of Tennessee from 1900 to present.

Birth Short
Please be advised that if you reside in the State of Tennessee and were born from 1949 to present, your local health department can provide assistance with birth certificates. The Birth Short copy is a certified transcript showing child's name, birth date, sex, county of birth, certificate number and file date for births that occurred in the State of Tennessee prior to 1976. The Birth Short Certificate that is issued for events that occurred from 1949 to 1975 does not contain the parents' names. You will need to order a Birth Long Certificate if the parents' names are required. The Birth Short Certificate cannot be used to obtain a passport.
