Birth Short
Please be advised that if you reside in the State of Tennessee and were born from 1949 to present, your local health department can provide assistance with birth certificates. The Birth Short copy is a certified transcript showing child's name, birth date, sex, county of birth, certificate number and file date for births that occurred in the State of Tennessee
prior to 1976. The Birth Short Certificate that is issued for events that occurred from 1949 to 1975 does not contain the parents' names. You will need to order a Birth Long Certificate if the parents' names are required. The Birth Short Certificate cannot be used to obtain a passport.
