Who Can Order?
To obtain copies of Utah death certificates, you must be a member of the decedent's immediate family. Immediate family is defined as a spouse, child, parent, sibling, grandparent or grandchild.
If you are the registrant's legal guardian, you must provide certified/stamped court approved documents. Documentation can be faxed to 866.760.1898 upon completion of your online order.
If you are a legal representative, (attorney, physician, funeral service director, genealogist, or other agent of the subject or subject's immediate family who has court documents) you must provide a letter, on business letterhead, stating why you are requesting the document. You will also need to submit backup documentation (court documents). Documentation can be faxed to 866.760.1898 upon completion of your online order.
