Who Can Order?

Legal Representatives may also obtain the record if they posses Power of Attorney or Legal Guardianship of the person named on the certificate. A certified copy of guardianship appointment must be submitted to the Bureau before the order can be processed. All requests must include a copy of government-issued picture ID of the applicant.
