The New York City Department of Health, Office of Vital Records (New York City Vital Records) issues certified copies of New York City birth certificates (from 1910 to present) and New York City death certificates (from 1949 to present) for events that occurred in the five boroughs of New York City (Manhattan, Brooklyn, Queens, the Bronx and Staten Island). New York City divorce records must be ordered through New York State Vital Records. You may order copies of New York City birth and death certificates through VitalChek on an expedited basis.

  • Does the name on your certificate match the name on your credit/debit card?
    Women- first name must match; Men- first AND last names must match

  • Have you legally changed your name for a reason other than marriage, AND amended your certificate with the Corrections Unit of the NYC Dept of Health?

      IF YOU HAVE NOT AMENDED AS NOTED ABOVE: your order must be placed by mail or in-person*

    • Please allow an additional ten (10) to fifteen (15) business days for vault copies of NYC birth certificates, which require a more extensive search. Vault copies are used for Apostille/Authentication, International Adoption, Dual Citizenship/Immigration, or International Marriage
    • If your address includes a P.O. Box, please be aware that express ordering is not available. Express couriers, such as UPS or FedEx, will not ship to a P.O. Box.

    Who Can Order?
    You may order copies of New York City birth certificates for yourself or for a child, as long as your name appears on certificate*. Your name must also appear on the credit or debit card that is being used for payment.

    Where Can The Document Be Shipped?
